Description
5-methyltetrahydrofolate (5-MTHF) is the most active metabolite of folic acid, bypassing all of the enzymatic activation steps in the conversion of folic acid to the biologically active 5-MTHF. Directly utilised by the body, 5-MTHF supports healthy methylation, homocysteine metabolism, DNA replication and foetal development.

Prescribing Information:
- Supplementation with folic acid and 5-MTHF is recommended during pregnancy at an intake of between 400-600mcg per day.
- Some concern exists that supplementation at high doses of folic acid could mask a B12 deficiency state.
- Supplementation with 5-MTHF appears to side-step this potential problem. Since 5-MTHF can only be converted to 5,10-MTHF after participating in homocysteine recycling with vitamin B12, it may not mask a B12 deficiency because 5-MTHFis not active in DNA synthesis without the help of B12.
- At doses up to 50mg a day, GI complaints, insomnia, irritability and fatigue have been occasional side effects.
- Theoretically, high doses of folic acid might increase toxicity of 5-fluorouracil.
- There is some evidence that folic acid supplements reduce the efficacy of methotrexate in the treatment of acute lymphoblastic leukemia, and theoretically they could reduce its efficacy in the treatment of other cancers.
- Folic acid, folinic acid or 5-MTHF supplementation does not appear to interfere with methotrexate's anti-arthritic or anti-inflammatory activity.
- Folic acid can have direct convulsant activity in some people, reversing the effects of phenobarbital and primidone, and worsening seizure control.
